Health & Assessment Information
Purpose of collection: To assess your reported pain points, mobility limitations, and physical condition in order to generate and deliver your personalized exercise/recovery plan.
Source: Collected directly from you, via a Google Forms assessment questionnaire.
Disclosure: Shared with Google LLC, which hosts and processes the form submission on our behalf (see Google's Privacy Policy). We do not use this information for advertising or sell it to third parties.
Data collected: Self-reported information about pain, injury history, mobility, lifestyle, and related health/physical condition details that you choose to provide in the assessment.
Lawful basis: Where required (e.g. for EEA/UK residents), we rely on your explicit consent to collect and process this information, since it may constitute special category (health) data. You may decline to provide this information, though doing so may limit our ability to generate a personalized plan for you.

This Site is not intended for individuals under 18. We do not knowingly collect information from children. If you believe your child has provided us with Personal Information, contact us to request deletion.
